Medicines Partnership
EIBS and Hammersmith based em:seven have just completed the launch of the
Concordance Website - the new site for the Medicines Partnership. Based on the EasySite content management system, the website provides a constantly updated resource of information regarding the effective prescribing of medicines.
Medicines Partnership is a two-year initiative supported by the Department of Health, aimed at putting the principles of concordance into practice. Concordance is a new way to define the process of successful prescribing and medicine taking, based on partnership:
The patient and the health care professional participate as partners to reach an agreement on the illness and treatment
Their agreement draws on the experiences, beliefs and wishes of the patient to decide when, how and why to use medicines.
Health care professionals treat one another as partners, and recognise each other's skills to improve the patient's participation.
